A 72 YEAR OLD MAN WITH ANOREXIA

Chief complaint:
Anorexia

  1. Present illness:
  2. 72 year old man /from Tehran/neurosurgeon/presented with anorexia and 20 kg weight loss during one year.
  3. Followed by sudden sever weakness and brief loss of consciousness one year ago .he was admitted to ICU and received supportive care.
  4. SBP was low he was hydrated with normal saline and hydrocortisone
  5. TSH >100   levothyroxine was started
  6. He was discharged with a diagnosis of hypothyroidism

  1. Weight loss(20 kg)
  2. Fatigue(+)
  3. Weakness(+)
  4. Nausea and vomiting(-)
  5. Headache(-)
  6.  Memory impairment(-)
  7. Hypoglycemia(-)
  8. Libido loss(-)
  9. Orthostatic hypotension(-)
  10. Blurred vision(-)
  11. Hyper pigmentation(-)
  12. Hyponatremia(+)
